Form for Laptop

    Hi all,

    IT Managers asked me to knock up a form for the use of laptops by the reps. Have done a bit, serial number, software, signed for etc. Just wondered what if any you guys use? Templates etc.

    We use a combined user agreement and equipement form (with terms & conditions on the back). Let us know if you want to see it.
