Troubleshooting forms.

    When you get a call from a user regarding a troubleshooting problem there is like a form you fill in with customer name make of the pc, problem description etc.. Does anyone know where i can get these forms from? Or should i just make them myself? If i do decide to make them what info should i include apart from user's name, pc description, problem details? Thank you.
