You need to be holding your nose with thumb and forefinger of the same hand and cupping your palm across your nose and mouth for this to work (see when).

A patient is wheeled, on a gurney and unconscious, with a drip in his arm and an oxygen mask on his face into a ward in a hospital and parked.

He wakes, and after a while, turns to the Gent in the next bed and says (this is the point where you have to do the above)

(Now)"Are my testicles black?" (Stop)

"I'm sorry," says the Gent, " I've no idea... I don't think it's any of my business."

A Nurse walks past and the patient again asks (Now)"Are my testicles black?" (Stop)

The Nurse pulls back the covers on his bed and checks his credentials. " No" she says "they're not."

The patient, now exasperated by the whole affair, pulls the mask from his face and says "bugger this
